Privacy Policy | Terms & Conditions

PART I: PRIVACY POLICY

At Andersonville Med Spa, we are committed to protecting your privacy and safeguarding your personal and medical

information. This Privacy Policy outlines how we collect, use, disclose, and manage your information when you visit our

facility, use our website, or interact with our communication channels, including email, telephonic, and Short Message

Service (SMS) text messaging platforms.

Please note that as a medical spa provider, certain clinical and health-related information you provide is additionally

protected under federal and state medical privacy regulations, including the Health Insurance Portability and

Accountability Act (HIPAA), as detailed below.

1. What Personal Information Is Collected

We may collect several types of information from and about users of our services, including:

Identity and Contact Data: Name, date of birth, biological sex, email address, postal address, and telephone

number.

Clinical and Health Data (Protected Health Information): Medical history, skin type, lifestyle factors, cosmetic

concerns, current medications, allergies, treatment records, and before-and-after photographs.

Financial Data: Credit card, debit card, or other payment account details processed securely through our PCIcompliant

payment gateways.

Technical and Usage Data: IP address, browser type, cookies, and usage statistics regarding how you navigate our

website.

2. How Personal Information Is Used (Including Email & SMS)

We utilize your collected contact information (both email addresses and mobile telephone numbers) for the following

professional purposes:

Transactional & Operational Communications: To schedule, confirm, modify, and remind you of your clinical

appointments; to send pre-treatment or post-treatment clinical protocols; and to deliver transactional receipts or

billing updates.

Marketing & Promotional Communications: To send emails or text messages regarding seasonal promotions,

special events, skincare product launches, loyalty program updates, and exclusive med spa membership discounts.

Clinical Management: To maintain comprehensive medical records in accordance with medical board regulations

and HIPAA standards.

3. Who Personal Information Is Shared With

We do not sell, rent, or lease your personal information to third parties. Your information is only shared under the

following limited circumstances:

Authorized Service Providers: Trusted third-party vendors who assist us in operating our website, managing

Electronic Medical Records (EMR) systems, executing payment processing, and administering our email and text

messaging communications infrastructure. All such parties are contractually bound to preserve data confidentiality.

Clinical Supervision & Healthcare Compliance: Licensed medical directors, practitioners, and clinical staff

directly involved in managing or auditing your medical aesthetic treatment plan.

Legal Mandates: Where required by local, state, or federal law, court order, or regulatory enforcement actions.

Strict SMS Consent Non-Sharing Mandate (Carrier Compliant)

Notwithstanding any other clause within this policy, SMS consent and phone numbers collected for text messaging

purposes are not shared with third parties or affiliates for marketing or promotional purposes under any

circumstances. Your text messaging opt-in data is strictly confidential and reserved solely for the operational use of

Andersonville Med Spa.

4. HIPAA and Medical Privacy Compliance Notice

While standard web interactions fall under generic privacy guidelines, any health-related data collected during

consultations, intake processes, or clinical treatments forms part of your Protected Health Information (PHI) under

HIPAA. We maintain rigid physical, technical, and administrative safeguards to protect your PHI. We ensure that our

communication platforms, intake software, and backup structures meet or exceed federal encryption metrics. Please note

that traditional SMS text messaging and standard unencrypted email are not inherently secure channels for transmitting

detailed clinical data; therefore, our automated text/email communications are restricted to operational logistics,

reminders, and promotional alerts rather than detailed diagnostic or therapeutic medical records.

PART II: TERMS & CONDITIONS

These Terms & Conditions govern your access to and use of the services, website, and communication programs

provided by Andersonville Med Spa. By booking an appointment, executing an intake form, or interacting with our text

messaging and email communication programs, you acknowledge and agree to these terms.

1. Scope of Medical Aesthetic Services

All clinical procedures, injections, laser treatments, and aesthetic regimens offered at Andersonville Med Spa are subject

to professional medical assessment. An initial consultation is mandatory to determine clinical candidacy. We reserve the

right to refuse treatment if a procedure is deemed clinically inappropriate, unsafe, or contra-indicated for a patient.

2. Communications Terms of Service (SMS & Email)

By providing your mobile phone number and email address via our web registration forms, digital check-in tablets, or

other enrollment mediums, you explicitly agree to receive both transactional messages and marketing updates from

Andersonville Med Spa.

A. Short Message Service (SMS) Communications:

Expected Message Types: You will receive automated and manual text messages concerning appointment

confirmations, appointment reminders, pre-care or post-care clinical instructions, order alerts, account notifications,

and marketing or promotional campaign offers from Andersonville Med Spa.

Messaging Frequency: Message frequency varies based on your active scheduling patterns, treatment cycles, and

promotional engagement levels.

Message and Data Rates: Standard message and data rates may apply depending on your mobile carrier and cellular

plan. Andersonville Med Spa is not responsible for fees incurred from your provider.

SMS Opt-Out Protocol: You may opt out of receiving text messages at any time. To opt out, text STOP to any

have been unsubscribed from our SMS network. Please note that opting out may impede your receipt of immediate

appointment adjustments.

Cross-Reference Documents: Visit [Privacy Policy URL] for our privacy policy and [Terms URL] for our Terms of

Email Opt-Out Protocol: You can opt out of marketing and promotional emails at any time by clicking the

Required Standard Enrollment / Disclosure Language:

“By opting into SMS from a web form or other medium, you are agreeing to receive SMS messages from Andersonville

Med Spa. This includes SMS messages for conversations (external) as well as appointment confirmations, operational

alerts, and marketing promotions. Message frequency varies. Message and data rates may apply. See privacy policy at

[Privacy Policy URL]. Message HELP for help. Reply STOP to any message to opt out.”

3. Appointment, Cancellation, and Refund Policies

Because our clinical practitioners allocate specific blocks of time for personalized treatments, Andersonville Med Spa

enforces a strict reservation protocol. Cancellations or rescheduling requests must be submitted at least 24 to 48 hours

prior to your scheduled block, depending on the specific procedural profile. Failure to do so may result in a late

cancellation fee or forfeiture of your deposit. All sales of clinical procedures, package bundles, and injected materials are

final once administered. Retail skincare products may only be eligible for store credit or exchange if returned unopened

within a verified timeframe.

4. Patient Conduct and Clinical Safety Acknowledgement

Patients are required to provide complete, exhaustive, and entirely accurate medical histories during the intake process.

Concealing medical conditions, active prescriptions, or aesthetic interventions performed elsewhere can cause severe

clinical complications. Patients must strictly adhere to post-treatment guidelines provided by our medical staff to ensure

optimal healing and outcomes. Andersonville Med Spa disclaims liability for adverse outcomes resulting from a patient’s

failure to disclose relevant medical realities or follow post-care protocols.

5. Updates and Governing Law

We reserve the right to update or modify these legal instruments at any time without prior individual notice. Continued

interaction with our clinic or digital systems constitutes acceptance of the revised versions. These policies are governed

by and construed in accordance with the laws of the jurisdiction in which our corporate and clinical facilities reside.
